A New Services member will visit with you in your home setting to discuss the level of care and frequency that matches your needs. A Registered Nurse is available 24 hours a day, 7 days a week to discuss your options and to answer any questions you may have.

Personal Care Caregivers


There are varying degrees of one-on-one care when you choose Agape Home Healthcare for Medicaid Managed Long Term Services and Supports (MLTSS) and private duty care. As a licensed agency, we provide a broad range of services to meet our clients’ total care needs. While the descriptors below provide a brief overview of service for personal care, Agape also offers skilled nurses who are licensed to assist with more in-depth care needs. Our New Services team is available 24/7 to answer any questions you may have.


Certified Nursing Assistants (CNA) & Personal Care Aide (PCA)


CNAs and PCAs provide personal, non-skilled, hands-on care for the client in addition to socialization and transportation. Respite care is also a common support requested when a spouse or significant other is the primary caregiver. Often times clients wish to prolong their independence in their own home, along with the autonomy they’re used to. Our caregivers, at every level, honor this request and assimilate to the home environment without interfering or taking charge.

Physical Disability Support

Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our caregivers can assist you with daily tasks such as mobility, eating, exercising, and grooming.


Care for Dementia Patients

Our experienced and highly trained caregivers provide the highest quality of care to both patients and their families. Our goal is to help individuals and their loved ones live happy and fulfilling lives.

Care Plans

How does Agape determine the type of care I need?

  • RN Clinical Supervisors work with clients to determine the level and frequency of care. The RN Clinical Supervisor then creates an individual care plan for each specific client.

  • RN Clinical Supervisor communicates with Agape caregivers on client’s condition and care needs.

  • RN Clinical Supervisors may direct Agape caregivers to set medication reminders for a client.


Mobility & Range of Motion: Exercises for strength & safety, Transfers, Ambulation

  • Transport in client or caregiver vehicle

  • Socialization & Outings

  • Driving to appointments (for example: physician appointments, pre-operation appointments, or hair appointments)


Independence

How do I maintain my dignity and privacy as a client?

  • Agape Caregivers assimilate into the client’s home and environment to encourage continued independence, on a safe level, for clients.

  • RN Clinical Supervisor communicates closely with the client to involve them with determining a schedule for care and caregiver preference (based on a system of personality, skill level, and location).

  • Agape Caregivers provide the primary caregiver respite care.
